  • I am 35. I currently use Retin-A, vita C serum and alternate that with your DMAE cream. I am starting to use a soapless cleanser by Oil of Olay and would like to use a good moisturizer at night. What order should I apply the above and can I use a moisturizer at night w/the Retin-A?

    Answer:  yes, just apply the moisturizer last, a few minutes after the Retin-A. JG 9.1.06

  • I am only 21 but have puffy under eyes with what appears to be very thin skin. Blood vessels are becoming slightly visible and fine lines have developed. I currently use Retin-A 0.025% for Acne but as concerned with using it under my eyes as it might thin the skin more, making blood vessels even more visible. What would you recommend?

    Answer:  Retin-A will irritate the skin for a bit but works to slowly thicken it. JG 8.31.06

  • Dr. Garcia, I am 50 years old and have extremely dry skin on my legs and arms. My face is oily. I take a fish oil pill everyday and put lotion on everyday after I bathe. Can you tell me what else I can do? Also, I have a small red bump on my bottom lip that won't go away. Any idea what it could be? I did restylane in my lips a year ago, and that has been there about that long. Any chance it could be that? Thank you.

    Answer:  red bump could be a small hemangioma that can possibly be treated with a laser, as to the skin on your legs and arms, all I can say is you need to apply topical moisturizers more frequently. JG 8.31.06

  • When using moisturizer do you need a seperate moisturizer specifically for the eye area?

    Answer:  only if the skin is damaged and needs more care. JG 8.30.06

  • If I use retin-a do you think I would still need a toner?

    Answer:  I typically do not use toners. JG 8.30.06

  • When using a good skin care regimin do you need to use everything from the same line?

    Answer:  not always, they make lots of products for profit, usually a good skin cleanser and a moisturizer and sunscreen, and if older some other intensive care product. JG 8.30.06

  • If I use retin-A and vitamin C and a soapless cleanser - what would a good moisturizer be- Oil of Olay or L'oreal and what order do I put these on?

    Answer:  Oil of Olay is the one I like better. JG 8.30.06

  • Aveeno has come out with peptide products- what do you think?

    Answer:  have not seen which ones it contains so cannot say, peptide is a generic term and can mean lots of proteins not all of which will work and also it is important to have a certain amount of the active ingredient and if you do not it won't do much. JG 8.30.06

  • Between L'oreal and Oil of Olay- which one do you think for a 35 year old?

    Answer:  Oil of Olay. JG 8.30.06

  • What bleaching cream would you recommend for dark circles under the eyes? And how long would it be befoe I see a noticeable improvement if used regularly?

    Answer:  dark circles, unless they are brown spots from the sun will not respond to bleaching creams. JG 8.29.06

  • Hi Dr. Garcia...4 mos. ago you recommended photofacial/ipl treatments for my sun damage/brown spots. I've had two treatments on my face and one on my arms & hands, about three weeks apart. I'm 56 with TOO MANY years of baking in the sun. The results are AMAZING. Thankfully they took before-pictures because I've already forgotten how bad my face was. My hands are virtually spot-free!!!!!! (young looking again!) And I had MANY spots on my hands....I will go back for one more touch-up on a handful of stubborn spots (nose & forearm).....just thought your readers would be interested in my results!! Cost in Sacramento (LASER CLINIC) was $1200 and worth every penny! A small fortune will now be spent on sunblock and self-tanners! ;) Thanks for the great advice.

  • Does skin that has been weakened by steroid cream eventually grow back stronger? Is treatment needed?

    Answer:  the longer they have been treated, the less likely it is to regain its normal quality and at times not much can be done to repair the damage. JG 7.28.06

  • Can retin a acrually tighten skin some? If so, how long does it typically take for one to start seeing results?

    Answer:  it will help fine lines and it reaches maximum effect in 12 months. JG 7.28.06

  • Dr. G, what can I use to get rid of brown spots on my legs. Do you have anything or do I need some kind of prescription or can I get something over the counter?

    Answer:  Retin-A and hydroquinone creams, both prescription, mixed together is a place to start. JG 7.28.06

  • Hi Dr Garcia, What do you think of IS Clinical products-specifically the serums? I currently use your DMAE and your peptide cream as well as Retin-A. My skin looks pretty good, but it can always use improvement. My biggest complaint is lines around my eyes and dark circles. Anything else you can suggest?Thx.

    Answer:  in general a good product line but what you are using is already stronger. I am working on some new things for the dark circles but not ready for prime time yet. JG 7.24.06

  • Dr. I have alot of sun damage on my arms due to sun sun exposure, I also hav kp which i think may have develped because of the sun damage, never had it before, nor does anyone in my famiy, can you recommend anything that can help with the fading of some of the freckles and something that can help w. the kp, i know its not curable.

    Answer:  All I can say is glycolic acid products, maybe some Retin-A and definitely sunblock and stop going in the sun. JG 7.24.06

    Answer:  no need for a moisturizer on top of the cremas you are using. And if the DMAE has seperated, yes, mix it back up. As to spider veins, lasers and injections can be tried but they will not all go away and they will all return in time. JG 7.20.06

  • I recently got my face burned with an oil removing soap for acne since it dries my face to much i think so the burn stayed red and they peel and the crust fell off but a brown spot was left and i want to know if it will disappear or i need treatment or something to remove it faster

    Answer:  it should fall off and heal on its own, but at times some bleaching cream like 4% hydroquinone, a prescription, can help. JG 7.19.06

  • Hi Doctor, I have a brown spot on top of my upper lip that looks like a moustache. I've had it for about a year now and since then I've been using Aclaro on and off (depending on how bad/good it looks). For the first 10 months of using it, the spot did not completely go away, but at least made it a little pale and i could almost cover it up completely with makeup. Sometimes i wouldn't even use the cream at all for weeks since it didn't look that bad. But now, after a year of using Aclaro, it seems that it's getting worse. I'm afraid Aclaro is having the opposite effect now that I've used it for such a long time. I avoid sun exposure as much as possible, wear 30 SPF everyday, even hats, so I don't think the fact that it's summer now it's the reason why it's getting worse, or is it? Should I stop using Aclaro? Is there any other way of getting rid of this horrible spot that makes me look like a man?!?!?!

    Answer:  I have no experience with that product but I would suggest using a 4% hydroquinone with Retin-A together to help fade it along wth sunblock all the time.JG 6.27.06

I forgot to ask if applying hydroqinone before the procedure would be a great idea? how long before? and what strength?     Answer:  Ideally I would start you on it for 2-4 weeks before, twice per day. One week is OK if it is all you have. Stength is 4%. And yes, Life is great. JG 6.26.06

  • i put a fade cream on that had 2% hydroquinone in it and my face goes bright red straight away (but it doesnt hurt or burn)after 20 minutes or so my face is back to normal. is this supposed to happen or should i stop using it??thanks kirsty

  • Dear Dr Garcia, after a co2 or rhytec procedure what after skin care protocol is the best to help achieve the best possible results. I read products such as acquafor or squalene oil help the healing process. Which is best? what do you advise for your patients?. One more very important question there seems to be two schools of thought with skin care post laser. one is as soon as possible that you need to cleanse your face then apply vaseline type mixture at least twice a day to help eradicate old skin so the new skin can come through. The other is don't touch your face at all except to apply vaseline, do not cleanse as this may prematurely take the skin off, the longer the old skin or wound is on the better healing and less redness afterwards. Dr Garcia which protocol is correct? please need your advice. thankyou.

    Answer:  I prefer the squalene over the Aquaphor. We use those after the skin has healed enough, usually two weeks. Along with that we use sunblock for 6 months. My skin cleaning protocol is to splash water on the face and use a paper towel or wash cloth to press against the face and slowly peel the washcloth off, no rubbing, to remove the old ointment prior to applying the new ointment. JG 5.30.06

  • Do you think the value of using Ibedenol is as great as the advertising around it? does it really work? It is very expensive. What can you recommend at a lesser price to do the same thing?

    Answer:  I think they have hyped it to a great degree with little science behind it, they just have a huge advertising budget with all the money they made off of Botox. I think creams with CoQ 10 have more science behind them as that is where idebenone comes from. JG 5.29.06

  • Hello Doctor Garcia, Is it really necessary to use eye cream around the eye area? Is is just as beneficial to use for e.g. for very dry skin to use a vitamin E cream for face and use that for the eye area as well. Do you have any suggestions? are toners a good idea for skin after cleaning. Thanks a bunch.

    Answer:  creams will help slow the aging process although it will not stop it. Toners are useful at times but not all people need it. JG 5.23.06

  • I've noticed that doctor's base the longevity of Sculptra from the 1st treatment, which doesn't make sense to me since it supposedly doesn't take full affect until 3 months later. So just i just subtrate 3 months from what ever time I'm given?

    Answer:  well actually, that is not totally accurate either. The longevity is based upon studies doen on HIV patietns where they were injected upto 6 times or more and then were found to need a touch up after about one year after the last injection with near total dissolving after two years. No one can say how long it really lasts because it will be different from patient to patient, some more some less. JG 5.17.06

  • I am 47 yrs. old and my acne seems to be getting worse. I seem to get this big underground cists in the chin area around ovulation and my cycle in the past, butthe past 2-3 months it almost seems weekly. I'm occasionally now seeing them appear around my nose area as well which I haven't had since teenage years! I have previously taken Mynocyclin, but don't like the idea of year round. Is there some natural way I can eliminate this problem?

    Answer:  good for brown spots and red spots, it is a series of treatments, and not permanent, in time and with sun they will all come back. We no longer offer it, cons are that it might not be as effective in removing the spots as promised and that the amount of collagen it creates is quite small and insignificant. JG 5.16.06

  • How many ipl treatments are typically required to remove light sunspots on the face?

    Answer:  varies form patient to patient but typically at least 3-6 or more. JG 5.13.06

  • What is your opinion of hide the years for crepe paper skin??

    Answer:  Retin-A, Rhytec or Co2 lasers. JG 5.12.06

  • What one procedure takes the most years off a womans face at the age of 53 in excellent health? Also have you seen bio-identical hormone replacement change the appearance of dry, mature skin?( The imbalance was lack of estrogen)

    Answer:  facelift. I do feel estrogens do help skin texture and quality also. JG 5.9.06

  • What is the difference between using ibedenol and ibedenone is an anti-aging creme. Is one better than the other for mature sun damaged skin?

    Answer:  I would not recommend either one, there are no scientific studies to show it works, only those done by the companies selling it. JG 5.8.06

  • I am 25 years old and have had back and facial acne scars what can i do to help erase these marks and also what helps erase stretch marks i have them on my under arm towards my arm pit please help thank you M.H 05/07.06

    Answer:  CO2 laser will improve the face ones but not remove them, the back ones will be minimally improved with a chemical peel and besides a laser to remove the redness, the stretch marks will not be gone and not much that can be done. JG 5.7.06

  • hello I had a mole removed about 7 years ago on my cheek, it left me with a scar--is there anything I can use now to diminish it? would even rejuv am/pm work??

  • I am a 27 year old asian who has had acne for 12 years now. My skin is very light, oily and sensitive. Although my acne is mild now, I still have some redness, discoloration and icepic scars on my cheeks, nose and forehead. My dermatologist is advising me to get 4-10 treatments of Laser Genesis (4wks apart). I am rather skeptical since this non ablative laser procedure is still new, and I haven't read much about it on the news and internet. Do you think this will improve my overall skin, and make my icepik scars less visible?

    Answer:  may help he brown spots but most likely not much for the ice pick scars. JG 5.6.06

  • How long does Sculptra last in your experience? I have read one to two years. I am interested in trying it, but am concerned about what I have read in terms of possible side effects that people have experienced (lumps). In your experience, has the treatment truly lasted two years? Thank you.

    Answer:  6-18 months, but touch ups are needed at least at one year. Lumps are possibly with any filler and the best way to avoid them is to inject deep and in small amounts. JG 5.6.06

  • What exactly is in the DMAE cream?

    Answer:  DMAE is a short for dimethylaminoethanol, a naturally occurring substance that facilitates the synthesis of a neurotransmitter acetylcholine. DMAE is a compound found in high levels in anchovies and sardines. Small amounts of it are also naturally produced in the human brain. An intriguing finding in some DMAE studies was that it reduced the accumulation of lipofuscin deposits inside cells. Lipofuscin is a cellular pigment consisting of aggregated chunks of molecular waste. It tends to occur in the cells of older people. It is likely that lipofuscin is not simply a byproduct of aging but also contributes to the aging process. Neurons, heart and skin of older people usually contain particularly large amounts of lipofuscin. It has been demonstrated that DMAE causes some degree of skin tightening. However, despite speculation it remains unclear how DMAE firms the skin -- whether by stabilizing the membranes, boosting acetylcholine, reducing lipofuscin deposits or none of the above. Whatever the mechanism, the effect of DMAE is often noticeable. Besides, even though DMAE can't fully reverse the existing facial sag, it may reduce its further progression.   • dr Garcia, I am already a -patient of yours and was wondering what the difference between radiesse and restalyn is? i did restalyn for my laugh lines, however after 2 months the lines are vivid again

    Answer:  Radiesse can be used but it at times leaves hard lumps that need to be cut out. Fillers may not be an option in your case if they do not last long enough.JG 5.6.06

  • About Sculptra - does it acutally lift the face while augmenting or does it just stretch out the skin where injected to make room? I understand that it's being called the liquid facelift, and in pictures it seems to lift (even the jowls) but didn't know if that was true.

    Answer:  it fills in lost tissue and when you lose tissue it can allow the skin to drop but it is not a facelift in a needle. JG 5.2.06

  • I've had one treatment of V-beam w/levulon Acid at the lower setting to treat mild cystic acne and enlarged pores. It has been 4 weeks since the procedure. I noticed a small improvement the week or so after the procedure but still have new cysts forming. In your opinion, does this need several procedures (at a higher setting?) or do i need to take another approach?

    Answer:  I doubt that a single session will clear it up, it is usually a series and the stength can be increased as your skin tolerance is discovered. JG 5.1.06

    Answer:  I think it almost as good as CO2 and much better than the other lasers, but amazing, that all depends on one's expectations. No matter what is done, you will have scars, but they will look better. JG 5.1.06

  • I had co2 laser under my eyes 4 weeks ago and am still red. I can't apply makeup because of the color and texture of the skin, which is waxy. What can I do at this point to address the color and texture? My Dr. prescribed a steroid cream, but I feel that it has only made the area more red.

    Answer:  I am sorry but I am not a big fan of Fraxel. Lots of hype, lots of promise, great resutls in a small percentage of patients, most patients see mild results, and many see no results. I seriously doubt it will do anything to shrink pore size due to the fact that if it works it will pull on the pore opening and potentially make it larger. As for collagen formation and skin tightening I prefer a Rhytec Portrait laser but it will not do anything for the pores either, sorry. JG 4.22.06

    Answer:  I think IPL alone or Obaji or microlaser will do well for the brown spots, the IPL will help with the Rosacea, the others probably will not and the Levulan will do nothing for the Rosacea and the discolorations will be better but you wil have lots of downtime compared to plain IPL. Realize also this will be a life long thing you will need to treat and there is not a single treatment that will control it once and for all. You will need to be into to it for the long haul and many treatments.JG 3.28.06

I am thirty-nine years old with olive skin, large pore and acne scars (mexican decent) and I have been using the obagi skin care line off and on for the last 11 years with good results but now I have this problem with my chin are and hyperpigmentation, I've used other bleaching creams other than hydroquinone and the dark spot goes away temporarily and then comes right back on. I put on 45 sunscreen that I buy at a doctor's office but still no help. Can you recommend any other type of skin bleaching for that area, for some reason that is the only place I get the dark spot.   • I would like to try a bleaching cream for my upper lip and wanted to check and make sure. I have two different problems. The entire area above my upper lip looks as if it won't tan (it's always lighter then the rest of my face) but it's not that noticeable. I've noticed that I'm also getting indentions from the dynamic winkles which might be casting shadows.     Answer:  As to a filler I think it will help, I may not be as convinced that the bleaching cream will help though. Bleaching cream sold over the counter are not strong enough and the doctor doing the filler will have his/her favorite material. Costs vary from surgeon to surgeon. Side efects are swelling, bruising and at times small lumps. JG 3.18.06

    Answer:  it is designed to be used around the eyes as it is stronger than the regular face cream, If someone needs a stonger cream, the eye cream can be used for the entire face. As to when you will see the difference, everyone is different. The worse the wrinkles are the sooner you see inprovement but when one is not too bad the improvements are not as quick as there is less that the cream can do. Also need to stay out of the sun, use sublock and stay away form smoking. JG 3.5.06

    Answer:  well, the honest truth is, all of them will probably improve it to a certain degree, but the most important thing to understand is that no matter what you do, it will come back, maybe not all, but probably most, with ANY sun exposure.     Answer:  I don't think collagen pills will do much, your stomach will just digest it like a protein. I think excess caffeine can dehydrate your skin but not a cup or two of coffee per day. JG 1.6.06

It is impossible to say how long it would take and also impossible to say whether it will take it all off but one thing I can guarantee is that eventually all the discoloration will come back unless you wear sunblock all the time.     Answer:  all treatments are mild and will get a small amount of collagen formation in the skin but will do absolutely nothing for the tissues below the skin. JG 12.18.05

    Answer:  lots of ads but very little to no science behind it. CoQ 10 has science in terms of positive effects on skin and Idebenone is a form of CoQ 10 though. JG 12.28.05

  • I had botox for the second time for under-eye wrinkles. The first time (six months ago) it was great. I loved the effects. This time, however, something is terribly wrong. It has left me with very dry, crinkled (literally) skin under my eyes that wasn't there before. I re-visited the Doctor who did the injections....she sait it seemed to her that it is working. I constantly keep extra moisturizing lotion under my eyes. I am getting re-married soon and I could just cry. Any advice? Thank you.

    Answer:  Botox Cosmetic should never be used for wrinkles on the under eyes as it will weaken the muscles. I think you should stop any retinol or Retin-A products or glycolic acid products and use some good moisturizing creams like Kinease or something like that. JG 11.14.05

    Answer:  glycolic first, then the DMAE.Our DMAE cream can be used on the lips too.The packaging is different but the material inside is the same. I think EMU oil is a great moisturizer and any good moiturizer will help buld new collagen. JG 11.09.05

  • I just started using your DMAE face cream. I have been using 10% glycolic acid lotion at night and I have a 30% glycolic peel every 60 days. Can I continue to use the 10% glycolic at night and just use the DMAE in the daytime? Would I not get as good of a result using DMAE just once a day? I also have your Neuropeptide cream. Using the 10% glycolic acid makes the 30% treatments easier.

    Answer:  You can but I think the DMAE is better than the glycolic acid product for aging skin. The peels are OK to continue. You can certainly mix them if you desire though. JG 11.08.05

  • Can you please explain exactly what it is that smoking cigarettes does to make you age badly? Does it kill the collagen, reduce the moisture, cause the actual structure of the skin to break down, kill the fat cells etc.? Just wondering.

    Answer:  it shrinks the blood vessels and there is premature breaking of elastin fibers and slower replenishing of lost collagen, it also induces damage to the DNA of the skin cells making proper cell growth abnormal and deficient. JG 11.07.05

    Answer:  Glycolic acid products help. A product called Tend Skin helps, and you may consider laser hair removal to get the hair to be thinner and lighter and less likely to occlude. JG 11.03.05

I've been to two consults one recommending 5 sessions each with Palomar Rejuvelux and alternating microdermabrasion and the other recommending Aurora RF sessions. I'm concerned about both the microdermabrasion and the RF with rosacea. Are these procedures typically bad for skin with rosacea?

    Answer:  I agree, typically I would suggest just an IPL or ND:YAG laser as the RF and microderm can aggravate the rosacea in most people.

    Answer:  you can use it with Vitamin C although ours has Vitamin C already. I woudl not mix it with Retin-A.DMAE is a short for dimethylaminoethanol, a naturally occurring substance that facilitates the synthesis of a neurotransmitter acetylcholine. DMAE is a compound found in high levels in anchovies and sardines. Small amounts of it are also naturally produced in the human brain. An intriguing finding in some DMAE studies was that it reduced the accumulation of lipofuscin deposits inside cells. Lipofuscin is a cellular pigment consisting of aggregated chunks of molecular waste. It tends to occur in the cells of older people. It is likely that lipofuscin is not simply a byproduct of aging but also contributes to the aging process. Neurons, heart and skin of older people usually contain particularly large amounts of lipofuscin. It has been demonstrated that DMAE causes some degree of skin tightening. However, despite speculation it remains unclear how DMAE firms the skin -- whether by stabilizing the membranes, boosting acetylcholine, reducing lipofuscin deposits or none of the above. Whatever the mechanism, the effect of DMAE is often noticeable. Besides, even though DMAE can't fully reverse the existing facial sag, it may reduce its further progression. Some people report a cumulative effect with continued use of DMAE. JG 10.17.05

    Answer:  Sorry but I think they are a waste of time and money. There is no product that can be applied to the skin that will relax muscles which are what causes the wrinkles. If there were it would be classified as a drug and would require a prescription. Application of a good DMAE and Vitamin C product will plump the skin and help it look better not only short but also long term as it plumps the skin. JG 9.27.05

    Answer:  I have never heard of that product so I cannot advise you. I think your best bet is to actually physically see a dermatologist or plastic surgeon so they can examne you and give you specific recommendations. JG 9.21.05

  • I have some sculptra bumps injected with some steroids. The bumps went down (barely) but now I have an indent around the bump. I understand that's the tissue loss is from the steroids...will the tissue grow back? (Some people say it will grow back in 6 months).

    Answer:  Whatever is going to grow back will grow back in 6 months. After 6 months you will need to decide if anything needs to be done or if it has corrected itself enough to leave it alone. JG 9.20.05

  • I an oriental person prone to scarring easily. In 1981 I had a series of corticosteroid injections into a post-surgical hypertrophic scar. This flattened the scar but has left the skin very thin and with multiple fine ridges/wrinkling and a red appearance. A dermatologist suggested that the affected skin is lacking elasticity (similar to aged skin), and the red appearance is due to underlying blood vessels being visible through the thinned skin. Are there any 1)products to reverse that corticosteroid effect now some 24 years later, e.g. testosterone cream or 2)procedures, both 1) & 2) to resurface or regrow the normal thickness of the affected skin.

  • What is the difference between Retin A and Renova? How often are you supposed to use the Renova and is there any area that you are not supposed to used the Renova?

    Answer:  Renova is the same material but in a cream base so it does not irritate as much but will not be as effective in wrinkle control. The only area you need to be careful is to not get it into the eye. JG 9.10.05

  • Hi Dr Garcia, I had wrote about a 2months ago about being on retin a and blender and skin turned too oil and all lumpy after 1year. yousuggested that i take a break for month or two. now iam wondering if it was caused by the retin a being cream form instead of gel form that made me too oilyand lumpy? they are switching me too retinamicro gel and blender. do you think that will fix the problem? or should i still take abreak and stay on what i was on the cream and blender? thank you very much

    Answer:  no, they both can do that. I think the Retin-A micro is less effective and may be easier for you to use. I think it is worth trying the new combination. JG 9.7.05

I really would like to stop taking it altogether, but have asked about doing so and never seem to get an answer about that either. I don't feel any differently taking it.     Answer:  Most people that treat thyroid disease are not familiar with anything out of that field. I think getting a second opinion would be a good idea. I think DMAE would be of benefit in your case but would be a lot to place all over your body. JG 9.5.05

  • I have been using Renova my Dermatologists gave me. I have had broken capilaries on my nose and chin lazer treated and they just come back, in the meantime I recently read that retin a and renova cause broken capilaries - Is this correct? and if so why would my Derm treat my capilaries and then give me something that causes them?     Answer:  Retin-A and Renova can in some individuals with thin skin cause an exacerbation of the small red capillaries. In most people, that can be transient as anything that thins the skin will increase them, but if the skin thickens in the long run they should get better. JG 9.1.05

  • Do you know if lachydrin will smooth out wrinkles, i tried it years ago (am a homecare nurse) on a patient with horrible skin dr dry and wirinkly on the legs d/t PVD and his skin became shiny and smooth after about 1 month- can i try it on the wrinkles in my neck?

    Answer:  Only very superifical ones that are recent. It will only improve them because it will rehydrate the skin, but not much more. So if your skin is not dehydrated, it wil not do much. JG 8.27.05

    Answer:  It probabaly is but the effects will not be as dramatic as on your face. We have alpha hydroxy acids in our skin creams but most good quality skin care lines now carry it as it is not a prescription. Thanks for the kind words. JG 8.26.05

  • Thanks for the opportunity to participate in this qustion and answer session. I was using Benefiance eye cream by Shesheido. I was very pleased and the jar lasted 2 years. When I replaced the empty I began to experience dryness, irritation and peeling of the skin under my lower lid. I tried the Obagi C system for 4plus months and developed eczema.Did not help the eyes at all. I tried Channel eye cream and it did well on my upper lid. However, the irritation returned under my lower lids. They are crinkly, and look as though they have slight fluid in the tissue. I have switched to Orlane eye cream and the skin is smoother. However, the concealer I use to cover dark circles under my eyes looks terrible. Any ideas about what product to use on a 58 year old nurse's eyes? I am on Premarin if that should make a difference. The rest of my face is really not bad at all. I have always taken very good care of my skin and used what I consider quality products.I hate to spend any more money on crea

    Answer:  At times the companies will change the formula and they put in ingredients that can irritate the skin. I think you need to avoid anything that exfoliates like Vitamin C and alpha hydroxy acid products. As to OTC products I would suggest Loreal Eye cream, or our DMAE eye booster on our shopping cart.   • Can you tell me about the DMAE cream and how it works?

    Answer:  mostly by stabilizing membranes so the cell wall is more stable and avoids the effects of aging and sun. There are reports that it helps muscle tone too but the research is still progressing on that front. JG 8.24.05

  • I was prescribed Tazorac .1% gel for acne. I was wondering if it would also help with my wrinkles. Also, I just finished acutane about a month ago. Is it safe to use Tazorac so soon? I do still have some breakouts. Thanks.

    Answer:  It might help with wrinkles and you need to watch your skin as it will be sensitive and if it gets too red or flaky please call your doctor. JG 8.24.05

  • Have you ever heard of Cosmelan, it's a depigmentation treatment manufactured by mesoestetic? If so, do you recommend the product and are there any long term side effects.

    Answer:  It has ingredients that are known to help with hyperpigmentation. I do not see any of its ingredients that should cause any long term negative effects. JG 8.13.05

  • I quit smoking five years ago. I'm 34 and suddenly, i have a ton of wrinkles all over my face. I have rosacea and its getting worse. Everything i've tried thus far has made my wrinkles, redness, dryness, pores, and facial flushing worse. My skin feels like it is on fire most of the time. All these failed treatments are causing me to lose hope. I have recently been prescribed finacea and Obaji skin care. I tried it for two days and my skin went nuts. (sorry for the long background). Do you have any thoughts at all? And, thanks in advance for any consideration.

    Answer:  I think you need to see a dermatologist or plastic surgeon. It is hard for me to comment without knowing what you have tried.I would suggest some mild IPL fotofacials and then progress to stronger ones once your skin can tolerate it. Best of luck. JG 8.13.05

    Answer:  I recommed that patients take a break form those products for a month or two every year. Prolonged use can at times cause some skin irritation and casue excessive skin oil production in an attmpt to moisturize the skin. It could have been caused by either the blender or the Restin-A. As one's skin gets better you need to understand that you need less strong medicine and continued use of strong medicine can cause that. JG 8.7.05

  • I am a 41 year old female with extensive sun damage and addtional skin damage caused from 10 years of smoking. I have recently stopped smoking, am currently using the Obagi Nu-Derm system as well as receiving microdermabrasion once a week. My dermatologist recommended an Obagi Blue Peel after 6 to 8 on the Obagi/Microdermabrasion...my question is, will this line of offense truly reverse all the abuse I have inflicted upon my skin.   • Thank-you for taking my question, Dr. Garcia. I have been using the Obagi Nu-Derm system for about 20 weeks. Is there a time when I should go off of the steps 3, 4, and retinol with 5? I was hoping to be able to stay on it for the rest of my life if need be. I use each step religiously every day. Also, I think I made a mistake and used the retinol over my eyelids as well. They are kind of crepey looking and I was wondering if that would improve if I stopped using the product on my eyelids. Have I done any permanent damage?     Answer:  I have my patients go off the retinol for 3 months a year if possible, usually the summer months, then restart everything again. I don't think you have done anything permanent to the eyelids but typically they are too thin to use the retinol on and can burn your eye if used so I would advise against it. JG 7.5.05
